Validation Visit Request Submission Form for Programs in Self-Study

The Request for Validation Visit Submission Form is now available for online completion and instant submission. This form is for use only by programs that are not currently accredited and are actively in Self-Study. Complete the form below. Once you have filled out all the required questions and uploaded your document(s), there will be an opportunity to review your form prior to submission. Upon clicking Submit, you will receive immediate confirmation of successful submission via a Thank You page and you will receive an emailed copy of your submission form. If you have any questions regarding your submission, please contact the Accreditation Office.

Note: Payment for the Validation Visit Fee is due with the submission of this form. If you prepaid your fee, please enter your Coupon Code from the email you received from the Accreditation Office to apply your payment to your form.
