Once you have filled out all the required questions and uploaded your document(s), click Submit, and you will receive immediate confirmation of successful submission via a Thank You page and an emailed copy of your submission form. Payment is due with submission of the form. If you prepaid your fee, enter the Coupon Code that was emailed to you to apply your payment.
