Annual Report Submission Form
The Annual Report Submission Form is now available for online completion and instant submission. Complete the form below. Once you have filled out all the required questions and uploaded your document(s), you will have an opportunity to review your submission before clicking Submit. You will receive immediate confirmation of successful submission via a Thank You page and an emailed copy of your submission form. If you have any questions regarding your submission, please contact the Accreditation Office.
Annual Report – Required Questions
Section 1: Program Improvements in Response to Recommendations
Utilizing the program’s Decision Letter, respond to all Standards individually listed under the Annual Report section, both Program Wide and Classroom Observations. Provide specific details of progress made to further meet the Standard as well as future plans on how the program will maintain compliance with the Standard.
Section 2: Program Improvement to More Fully Meet Standards
Select three to five Standards of your choosing, not already included in Section 1, and provide a description of how the program has further strengthened the Standard over the past year.
Section 3: General Program Enhancements
Describe any enhancements made at the program over the previous year that do not necessarily pertain to a specific Standard. For example: kitchen upgrades, painting, playground or classroom improvements, etc.
Section 4: Ongoing Compliance with Standards
Provide specific details on steps taken to orient new staff with Accreditation Standards as well as ongoing practices used to help staff maintain compliance with Standards throughout the year.
Payment is due with the submission of your Annual Report. If you prepaid your fee, enter the Coupon Code that was emailed to you to apply your payment. If you are unable to pay by credit card, contact the Accreditation Office at [email protected] to arrange an alternate form of payment. Note: Your Annual Report will not be accepted nor reviewed until full payment is received.
